4. Payment, Fees, and Insurance

You are financially responsible for all services received. We accept cash, credit/debit cards, HSA/FSA cards, and participating insurance plans.

  • Please verify your insurance coverage before treatment

  • Copays and deductibles are due at the time of service

  • Outstanding balances must be paid promptly to avoid collection procedures

5. Cancellation & Missed Appointments Policy

We require 24 hours’ notice for cancellation or rescheduling of appointments.

  • No-shows or late cancellations may incur a fee of $35

  • Repeated missed appointments may result in scheduling restrictions

6. Privacy and Confidentiality

Your health and personal information is protected under HIPAA and our Privacy Policy. We do not share your identifiable health information without your consent unless required by law. By using our digital tools (such as online forms or Facebook Ads), you consent to the use of anonymized tracking tools unless you opt out.
